Job Overview

The Telehealth Care Coordination & Provider Support Specialist delivers clinical, operational, and care coordination support to healthcare providers delivering virtual Transitional Care Management (TCM) services. In this role, you will coordinate post-visit follow-up care, facilitate referrals, connect patients with community resources, and address individual care barriers. You will also review medical documentation to ensure regulatory compliance and optimal patient outcomes. This fully remote position requires a proactive professional capable of managing time-sensitive workflows in a fast-paced telehealth environment.

Schedule 1: Mon-Fri, 7:00 AM - 4:00 PM PST (40 hours per week)

Schedule 2: Mon-Fri, 8:00 AM - 5:00 PM PST (40 hours per week)

Responsibilities

  • Provide daily clinical, operational, and care coordination support to telehealth physicians and advanced practice providers during virtual patient visits.

  • Identify and follow up on patient needs resulting from provider assessments, care plans, and post-discharge recommendations.

  • Coordinate patient follow-up appointments with primary care providers, specialists, and other healthcare professionals.

  • Facilitate, send, and track referrals for home health, skilled nursing, therapy, durable medical equipment (DME), and community resources.

  • Communicate directly with patients regarding follow-up plans, referrals, appointments, and care education.

  • Liaise with external physician offices, specialist practices, home health agencies, and care partners to ensure seamless continuity of care.

  • Identify patient barriers to care (e.g., transportation, medication access, financial/social needs) and connect patients with appropriate community resources.

  • Review patient charts for documentation completeness, medical necessity, care plan requirements, and regulatory compliance.

  • Assist in educating providers on TCM, E/M coding, ICD-10-CM, CPT, and telehealth modifiers (93/95).

  • Support responses to audit requests (ADR, TPE, UPIC, CERT) and monitor provider quality metrics and documentation timeliness.

  • Collaborate with internal teams—including Coding, Quality, Credentialing, Care Management, and Operations—to optimize care delivery workflows.

  • Maintain accurate, detailed documentation of all patient outreach, referrals, appointments, and resource coordination.

Must-Have Requirements

  • Proven experience supporting physicians or advanced practice providers.
  • Demonstrated knowledge of telehealth workflows and Electronic Health Record (EHR) systems.
  • Strong background in care coordination, follow-up planning, and continuity of care.
  • Exceptional communication, organizational, problem-solving, and customer service skills.
  • Ability to work independently, manage time-sensitive tasks, and prioritize patient needs in a remote environment.
  • A dedicated, quiet home workspace free from distractions, paired with reliable high-speed internet.

Nice-to-Have Requirements

  • Prior experience in Transitional Care Management (TCM), virtual care management, or telehealth operations.
  • Experience coordinating home health, skilled nursing, or post-acute care referrals.
  • Familiarity with community-based resources and addressing Social Determinants of Health (SDOH).
  • Experience navigating care coordination for post-discharge patients across multi-provider healthcare settings.
