Working remotely with an experienced in-office nursing team, you’ll conduct essential clinical reviews that directly impact patient outcomes and safety. Client Overview
A well-established home healthcare company serving Northern California for over 10 years, providing essential nursing and rehabilitation services to elderly patients who cannot leave their homes. They focus on preventing hospital readmissions through proactive clinical review and patient monitoring, working with an established network of hospitals, physician offices, and nursing homes.
Schedule

Monday - Friday, 09:00 AM - 06:00 PM PST, with 1 hour unpaid break (40 work hours per week)

Responsibilities
- Conduct clinical reviews of referrals from hospitals, physician offices, and nursing homes to assess patient appropriateness and risk levels
- Evaluate cases to identify patients at high risk for hospital readmission and document findings
- Collaborate with in-office nursing staff to develop clinical pathways that prevent 3-day hospital readmissions
- Perform Friday “tuck-in calls” to all active patients as wellness checks to prevent weekend readmissions
- Document clinical assessments and recommendations in the company’s EMR system
- Communicate effectively with healthcare team members through company communication platforms
- Work closely with the authorization team to ensure seamless patient care coordination
Requirements
- Registered Nurse (RN) credential (US state licensure not required) - or any clinical experience
- Strong clinical background and healthcare experience
- Excellent communication skills for patient interactions and team collaboration
- Strong reading comprehension and clinical assessment abilities
- Bonus if you have experience with EMR (Electronic Medical Records) systems
- It helps if you’re familiar with home healthcare or patient care coordination
