Patient Care Coordinator

Job Overview

We are seeking an experienced Patient Care Coordinator with a background in U.S. medical practices to handle routine calls, portal messages, and clinical support requests. Key responsibilities include gathering patient info, accurately documenting communications, and routing requests—especially medication-related inquiries—to the appropriate care team members. Experience in pain management or medication-heavy specialties and strong knowledge of medical terminology are highly preferred. 

Schedule: Remote, Full Time | Monday–Friday8:00 AM–5:00 PM EST.

Responsibilities:

  • Serve as an initial point of contact for routine patient calls, portal messages, and clinical support requests.
  • Triage and appropriately route routine patient concerns to the appropriate clinical team member.
  • Handle and coordinate medication refill requests, pharmacy-related issues, and medication questions according to established protocols.
  • Assist with routine post-procedure questions and determine when concerns require escalation to a medical assistant, nurse, or provider.
  • Address appointment-related clinical questions and route requests appropriately.
  • Gather standardized patient information, including symptoms, medications, timing, relevant history, and other required details before escalating clinical messages.
  • Review incoming messages to ensure they contain complete and accurate information before forwarding them to the appropriate team member.
  • Document patient communications and actions accurately in the EMR/EHR.
  • Follow established triage protocols, workflows, and escalation procedures.
  • Identify urgent or potentially concerning patient issues and promptly escalate them according to practice guidelines.
  • Communicate with pharmacies, patients, providers, and other healthcare team members as needed.
  • Follow up on pending clinical messages and requests to help ensure timely resolution.
  • Maintain patient confidentiality and comply with HIPAA and applicable healthcare regulations.
  • Help organize patient communications so medical assistants and providers can focus on higher-level clinical care.
  • Perform other clinical administrative and coordination duties as assigned.

Requisites:

  • Previous experience in a patient triage, clinical support, medical assistant, patient coordinator, or similar healthcare role.
  • Windows PC with a processor Intel i5 or i7 or similar, 16 GB of RAM, and Internet Speed: 30 Mbps or higher.
  • Must have the flexibility to extend or adjust working hours when needed to support patient and operational needs.
  • Experience supporting a U.S.-based medical practice is strongly preferred.
  • Strong understanding of medical terminology and common healthcare workflows.
  • Experience handling patient calls, portal messages, medication requests, and clinical communications.
  • Excellent written and verbal communication skills with a proven ability to interact with patients professionally, empathetically, and clearly. 
  • Strong attention to detail, time management, and organizational skills, with a focus on accurate documentation, following protocols independently, and collecting key details to manage or escalate patient requests effectively. 
  • Proficiency with EMR/EHR systems, patient portals, Microsoft Office, email, and other healthcare technology.
  • Preferred: Prior background in medication-heavy specialties (such as pain management, orthopedics, rehab, or primary care), with specific experience managing refill requests, pharmacy communications, high-volume patient messages, and controlled-substance workflows. 
  • Preferred: Familiarity with U.S. healthcare systems, insurance processes, patient privacy (HIPAA) requirements, and practice-specific clinical escalation procedures.
