Overview
Our client is looking for a Care Coordination Team Lead – Home Health to provide frontline leadership to a team of Care Coordinators responsible for scheduling, staffing, and coordinating home health services. In this role, you will combine hands-on care coordination oversight with people management to ensure the right caregiver is matched to the right patient seamlessly. You will serve as the first point of escalation for complex cases and staffing conflicts while coaching your team on workflow, documentation, and customer service standards.
Schedule:
- Full-time 40 hours per week, Mon-Fri 9 am to 6 pm EST (New York). | Initial training and ongoing coaching.
- May require occasional evening, weekend, or on-call availability to manage urgent staffing gaps
Responsibilities
Team Leadership & Coaching
- Supervise, mentor, and coach a team of Care Coordinators, providing daily guidance on caseload management, prioritization, and problem-solving.
- Conduct regular one-on-ones and team huddles to review open cases, unfilled shifts, and recurring scheduling issues.
- Onboard and train new coordinators on scheduling systems, agency workflows, and communication standards.
- Monitor team performance against KPIs (fill rate, time-to-fill, missed visits, caregiver/patient satisfaction) and provide feedback or corrective coaching as needed.
Care Coordination Oversight
- Oversee day-to-day matching of caregivers/clinicians to patient cases based on skill set, availability, location, and payer requirements.
- Serve as the escalation point for last-minute call-outs, coverage gaps, and complex or high-acuity case placements.
- Review and audit scheduling accuracy and authorized service hours.
- Partner with intake, HR/recruiting, and clinical supervisors to ensure adequate staffing coverage across the caseload.
- Track and report on open shifts, pending referrals, and re-authorizations to leadership.
Communication & Customer Service
- Act as a point of contact for patients, families, and referral sources when issues are escalated beyond the coordinator level.
- Ensure timely, professional communication with caregivers regarding schedule changes, expectations, and agency policy.
- Document interactions, complaints, and resolutions accurately in the agency's scheduling/EMR system.
Compliance & Quality
- Ensure coordination practices align with state home health regulations, payer requirements, and agency policy.
- Support audits and quality reviews by ensuring scheduling documentation is complete and accurate.
- Identify workflow gaps or recurring compliance risks and recommend process improvements to leadership.
Requirements
- Associate's or Bachelor's degree in Healthcare Administration, Business, or related field preferred; equivalent experience considered in lieu of degree.
- 1+ year of experience in home health, home care, or healthcare staffing coordination, including at least 1 year in a lead or supervisory capacity.
- Working knowledge of home health scheduling systems (e.g., HHAeXchange, ClickUp documentation platforms).
- Familiarity with home health regulatory requirements (state licensure rules, Medicare/Medicaid home health conditions of participation, payer authorization processes).
- Demonstrated ability to lead a team, manage competing priorities, and stay composed under time pressure.
- Strong written and verbal communication skills; comfortable de-escalating patient, family, and caregiver concerns.
- Proficiency with Microsoft Office/Google Workspace, particularly spreadsheets for tracking staffing and coverage.
