Registered Nurse

Job Overview:

 

We are seeking a compassionate, detail-oriented Medical Assistant, Licensed Practical Nurse or Registered Nurse to join our growing healthcare team. In this fully remote role, you will ensure continuity of care between provider visits by monitoring patient health, assessing condition changes, and communicating key clinical findings to the care team.

 

Schedule: Monday Through Friday 8:30 AM – 5:00 PM Eastern Standard Time.

 

Responsibilities:

    • Monitor and maintain ongoing communication with patients between provider visits.
    • Perform routine patient assessments and identify changes in health status.
    • Escalate significant clinical findings or concerns to the supervising provider or clinical supervisor.
    • Document patient interactions accurately and thoroughly in the electronic medical record (EMR).
    • Support care coordination initiatives to improve patient outcomes.
    • Provide patient education and reinforce care plans as directed by the clinical team.
    • Collaborate with providers and other healthcare professionals to ensure timely, high-quality patient care.

 

Required:

    • Certification as a Medical Assistant, or an active LPN or RN license.
    • Excellent verbal and written communication skills.
    • Strong clinical assessment and critical thinking abilities.
    • Comfortable working independently in a remote environment.
    • Proficient with EMR systems and standard computer applications.
    • Reliable high-speed internet connection and a dedicated remote workspace.

 

Preferred

  • Experienced with: Remote Therapeutic Monitoring (RTM), Chronic Care Management (CCM), Telephone Triage
  • Bilingual in English and Spanish.
  • Previous experience in outpatient, primary care, or care management settings.
