Patient Intake Specialist

About the Role

We are seeking a compassionate, detail-oriented, and highly organized Intake Coordinator to support patients and families throughout the onboarding and care coordination process. This role serves as the first point of contact for new patients, helping gather important clinical information, coordinate schedules, and ensure patients receive timely support and appropriate care referrals.

The ideal candidate is empathetic, professional, and comfortable handling sensitive conversations while maintaining accurate documentation and confidentiality. Experience in mental health, healthcare administration, patient intake, or care coordination is highly preferred.

Schedule

  • Monday to Friday
  • 8:30 AM to 5:30 PM CST (Houston, TX)

Responsibilities

  • Coordinate patient schedules and appointment bookings.
  • Greet and interview new patients and families, build rapport, and provide initial support and guidance.
  • Collect detailed mental health histories, personal backgrounds, current symptoms, and patient goals for care.
  • Screen for urgent needs, assess immediate risks, and determine the appropriate level of care or intervention.
  • Schedule appointments with therapists or clinicians, coordinate care, and make internal or external referrals as needed.
  • Manage new patient records, perform data entry, and ensure all documentation is complete and accurate.
  • Maintain confidential and accurate patient files in accordance with privacy regulations and organizational standards.
  • Provide initial crisis intervention and support when necessary, ensuring patient safety and proper escalation.

Requirements

  • Previous experience in mental health intake, patient coordination, healthcare administration, or a related field.
  • Strong communication and interpersonal skills with the ability to build rapport with patients and families.
  • Excellent organizational and scheduling skills with strong attention to detail.
  • Ability to handle sensitive and confidential information professionally.
  • Comfortable conducting intake interviews and gathering patient information.
  • Ability to assess urgent situations calmly and escalate appropriately when needed.
  • Experience using EMR/EHR systems, scheduling platforms, or CRM tools is preferred.
  • Proficient in data entry and administrative documentation.
  • Compassionate, patient-focused, and able to work in a fast-paced environment.
  • Fluent English communication skills, both written and verbal.
