Reports to: Clinical Operations
As an Outreach Care Navigator focused on Gap Closure, Wellness, and Post-Acute Follow-Up, you will perform telephonic outreach to health plan members, helping close gaps in care, arrange wellness visits, and support post-discharge follow-up with primary care providers (PCPs) within 7 days. This role aims to improve health outcomes and reduce readmission risks through proactive engagement and effective communication.
Key Responsibilities
- Conduct telephonic outreach to members post-discharge, focusing on gap closure, wellness visits, and PCP follow-up within 7 days.
- Educate members on preventive and post-acute care, emphasizing the benefits of timely PCP follow-up to reduce readmission risks.
- Use KBRAX software for documenting member interactions, tracking follow-ups, and updating care status.
- Coordinate with clinical teams to maintain accurate member records and ensure proactive care support.
- Guide members on health plan benefits, preventive screenings, wellness programs, and available support to encourage health engagement.
- Maintain confidentiality and adhere to privacy policies, following regulatory compliance.
- Meet performance and quality standards as set by the Call Center Manager and quality team.
Required Knowledge, Skills, and Abilities (KSAs)
- Healthcare Knowledge: Familiarity with preventive care, wellness programs, post-acute care, and gap closure processes.
- Communication Skills: Excellent telephonic communication, with the ability to engage members empathetically and promote health engagement.
- Documentation Accuracy: Strong attention to detail in documenting outreach activities and follow-ups in KBRAX.
- Problem-Solving Skills: Ability to address member concerns and connect them to appropriate healthcare resources.
- Customer Service Orientation: Commitment to enhancing member satisfaction and improving health outcomes.
- Technical Skills: Proficient in KBRAX software (preferred) and other call center tools.
- Computer Literacy: Basic computer skills, including MS Office and Teams.
Qualifications
- Relevant healthcare certification or experience in a healthcare call center, care coordination, or outreach role.
- Experience with telephonic outreach, gap closure, and wellness scheduling preferred.
- Strong English communication skills, both verbal and written.
- Ability to work remotely with reliable internet and VOIP setup or in a call center setting, if required.
